Gift Card Vouchers

The following program vouchers are for eligible Medica Choice Care^SM or Medica MinnesotaCare members only. Please refer to the age requirements listed for each.

Print and bring a copy of the appropriate voucher to your doctor at the time of your appointment.

• Best for Baby & Me Prenatal Care Voucher No. 1: Earn a $20 Target^® GiftCard when you have your first prenatal visit within 104 days of becoming pregnant.
  
  
• Best for Baby & Me Prenatal Care Voucher No. 2: Earn a $60 in Target^® GiftCards when you receive certain prenatal visits and a postpartum visit.
  
  
• Blood Lead Test Voucher: Earn a $50 Target^® GiftCard when your baby has a blood lead test between the ages of 9 months through 30 months.
  
  
• Breast Cancer Screening Voucher: For women ages 40 to 75, earn a $15 Target^® GiftCard from Medica when a mammogram is performed.
  
  
• Cervical Screening Voucher: For women ages 21 to 64, earn a $15 Target^® GiftCard from Medica when a Pap test is performed.
  
  
• Child and Teen Checkups (C&TC) Incentive Voucher (Birth through 14 months): Earn a $75 Target^® GiftCard when your child completes six Child and Teen Checkups (C&TC) visits between the ages of birth and 14 months.
  
  
• Child and Teen Checkups (C&TC) Incentive Voucher (15 months to 20 years): Earn a $10 Target^® GiftCard when your child completes a Child and Teen Checkups (C&TC) visit between the ages of 15 months to 20 years.

Other Programs

Car Seat Program

Medica will provide an infant/toddler car seat to women who are at least 6 months pregnant and children 2 years and younger.  In most areas, Medica has contracted with local public health agencies and/or clinic staff who will give the car seats to members.  They will also provide education on how to properly use the car seat.  We will mail a car seat to members who live in areas where we do not have a contracted car seat agency.  To request a car seat, call toll-free 1-866-455-7426.
