Provide-A-RideSM: Medica Choice Care Transportation Service
We provide transportation at no cost for Medica Choice Care members to and from medical, dental, mental health and substance abuse visits. To schedule transportation:
- Call Provide-A-Ride at least two working days before the appointment
- Be ready to give your member ID number and the doctor's full name and address
