• Most plans list coverage for a Routine Exam benefits will be found in the Professional/Physician Services section of your benefit document. It will be paid under the early disease detection services including physicals section.

 
Things to remember

• If during your visit, a condition is diagnosed, monitored, or treated you will be responsible for an office visit regardless of why you scheduled the appointment. Your office visit benefits are found in the Professional/Physician Services section under office visit.
• If you receive a prescription to treat a condition or an ongoing prescription is renewed during your visit, you may be responsible for an office visit charge. Your office visit benefits are found in the Professional/Physician Services section under office visit.
• If you have any further questions regarding your benefits, please contact Customer Service at the number on the back of your ID card.
