Medica Choice® Passport
Members with Medica Choice Passport participate in one of the following two networks. Please refer to materials provided by your company's Benefits Administrator to determine which network will apply to your coverage.
** Please confirm with the provider's office that they are in your plan’s network before your first and each subsequent appointment. If you have questions regarding whether your provider or clinic participates in your health plan, your benefits and/or coverage, please call the Customer Service number on your member ID card.
