A routine eye examination that measures a person’s ability to see an object at a specific distance performed when there is no existing complaint about the patient’s health.

Where do I find out how this will be covered with my benefit plan?

  • You will find your benefits in the Professional/Physician Services section of your Benefit Document. It will be paid under refractive eye exam.
  • Some plans do not cover refractive eye exams. If you do not see refractive eye exam in your benefit document, you do not have coverage for this service.

 
Things to remember

  • Most plans carry a limit on the number of visits for refractive eye exams. This will be listed in the same place where you find your benefits.
  • If concerns or issues about your eyes are talked about during your refractive eye exam such as cataracts or glaucoma, you may be billed for an office visit as well. The office visit benefit will also be found in the Professional/Physician Services section of your Benefit Document. It will be paid under office visit.
  • For most plans, glasses, contacts, and any related fittings are not included in your benefits.
  • If you have any further questions regarding your benefits, please contact Customer Service at the number on the back of your ID card.
