You can file a complaint, grievance or appeal in writing, or by telephone. Call Medica Customer Service at the appropriate number listed below. You may also use the address at the bottom of this page. If you make a verbal complaint/grievance that is not completed to your satisfaction, Medica will inform you of your right to submit your complaint/grievance in writing. Medica will also inform you of this right if we are unable to resolve your concern within 10 days. For assistance in completing the complaint/grievance form, contact Customer Service.
