Individual and Family Plan Members
Thank you for choosing Medica as your health plan! We want to help you maximize your benefits and your health by getting you started on the right foot. Health needs can crop up at unexpected times. Here are some action steps you can take right now so you’re ready to use your plan whenever you need it:
Watch your mail for your ID card and your member welcome kit. Both will arrive within the month of your effective date; the ID card usually arrives first. Review your ID card to verify all personal information is accurate. If you don't receive your ID card, please call Customer Service at 952-992-1805 or 1-866-894-8051; TTY 952- 992-3650 or 1-800-234-8819.
The name of your provider network will also appear on the card. You will need to select this network name when you use our online provider search tools.
Be sure to carry your ID card with you at all times so it’s available whenever you need care.
When you receive your plan information, it is important to review all documents to understand how the plan works and your financial responsibility, including copays, coinsurance and out-of-pocket costs. Be aware of how your out-of-pocket costs might change if you get care from a provider who is not in your network, or how you’re covered if you need care while traveling.

Our phone lines are open Monday/Tuesday/Thursday/Friday from 7 a.m. - 6 p.m. and Wednesday from 7 a.m.-6 p.m. (closed 8 - 9 a.m.). If you haven't received your ID card, please call 952-992-1805 or 1-866-894-8051; TTY 952- 992-3650 or 1-800-234-8819 for assistance).
- CallLink Nurse Line: Call 1-800-962-9497 to talk with professionals who can offer advice and answer your health care questions. Our nurse line is available 24 hours a day, seven days a week.
