Watch your mail for your ID card and your member welcome kit. Both will arrive within the month of your effective date; the ID card usually arrives first. Review your ID card and note that you have a subscriber number (unique to you) and a group number (unique to your employer). If you don't receive your ID card, please call Customer Service at 952-945-8000 or 1-800-952-3455; TTY: 952-992-3190 or 1-800-841-6753.
The name of your provider network will also appear on the card. You will need to select this network name when you use our online provider search tools.
Be sure to carry your ID card with you at all times so it’s available whenever you need care.
When you receive your plan information, it is important to review all documents to understand how the plan works and your financial responsibility, including copays, coinsurance and out-of-pocket costs. Be aware of how your out-of-pocket costs might change if you get care from a provider who is not in your network, or how you’re covered if you need care while traveling.
If you have a plan with a Health Reimbursement Account (HRA) , Health Savings Account (HSA) or Flexible Spending Account (FSA), be sure to become familiar with the process for filing claims.
You can register to view personalized plan information and your claims online with myMedica.com. This site will allow you to view all processed claims on demand, as well as provide your Certificate of Coverage and benefit information specific to your plan.
