Health Club Reimbursement Program

Fit Choices by Medica is a health club reimbursement program in which Medica provides up to a $20 credit towards health club membership monthly dues when a member meets the monthly attendance requirement at any network fitness club.

Where do I find out how this will be covered with my benefit plan?

• To see if you are eligible for the Fit Choices by Medica program, call Customer Service at the phone number on the back of your ID card or check with your employer.
• Find a participating fitness facility.

Things to remember

• You will want to enroll in the program at your health club. Your health club will administer the entire program.
• Your health club will keep track of how many times per month you visit the club.
• Ask your health club about how the reimbursement works, as it depends on the club as to how they administer the reimbursement amounts.
